Requirements
Please note that the following items are required from each player before the registration process can be completed:
- Color headshot photo.
- Copy of birth certificate or passport – not necessary if your child played for PA Soccer Academy last year).
- Completed PA Soccer Academy Medical Release (see below).
- PS1 – check ADD and complete as much information as possible (see below).
- 2018-2019 RG6 (see below – please do not forget to sign).
